How do I apply for Food Assistance benefits and receive an Ohio Direction Card?

In order to apply for food assistance an applicant (or their authorized representative) must complete an application online at or complete a general application (7200) and submit to the agency in person, by mail, or by fax. Once the application is received, the applicant will be scheduled for a telephone interview or a face-to-face interview with an Eligibility Referral Specialist.

Once eligibility has been determined, an Ohio Direction Card will be issued to the applicant by the vendor responsible for the cards and should be received at the applicant’s address within seven to ten days. Please keep your Ohio Direction Card, even if you become ineligible for food assistance. The Ohio Direction Card can be used with food assistance that you may receive in the future.

If you have previously had an Ohio Direction Card and have lost your card it will be necessary to contact the Ohio Direction Card Customer Service number at 1-866-386-3071 in order to request a replacement card. This number can be contacted 24 hours a day, seven days a week. The county agency no longer issues cards and all replacements must be requested from the vendor at the number listed.
